Maxim Street Health Hub is a mixed-billing practice, which means an out-of-pocket fee applies to most consultations. The table below outlines estimated costs based on appointment duration and complexity. Please note that each practitioner operates independently and billing is at their individual discretion. Standard Mixed Billing Policy

Practitioners: Dr Anna Ahn, Dr Tracey Aguilar, Dr Allan Chain, Dr Benjamin Chua, Dr Guy Davies, Dr Anna Pick, Dr Shanthini Tambimuttu and Dr Judy Xu.

Bulk Billing Eligibility (Monday – Friday): Bulk billing is available for weekday consultations for:

• Children aged 17 and under

• Health Care Card (HCC) holders

• Pensioner Concession Card (PCC) holders

• Nurse-led appointments (e.g., simple injections, flu vaccines)

• Chronic disease management (e.g., GP Management Plans and health assessments)​

Please note: Bulk billing is not available on weekends. Standard out-of-pocket fees apply to all weekend consultations.

Individual Doctor Billing Policies

The following practitioners operate under individual billing policies that differ from our standard mixed billing policy:

• Dr Joon Kwon
  Bulk bills children aged 15 and under, as well as HCC and PCC holders. All other patients are privately billed.

• Dr Natasha Ting
  Bulk bills children aged 17 and under and PCC holders only. All other patients (including HCC holders) are privately billed.

• Dr Rosanna Chan & Dr Maria Da Costa
  Bulk bill children aged 17 and under, as well as existing PCC and HCC holders known to the doctor. All new adult patients (18+) are privately billed.

• Dr Jennifer Jiang
  Bulk bills children aged 15 and under only. All patients aged 16 and over are privately billed, regardless of concession cards.

• Dr Camilla Holt
  Fully Private Billing: All patients are privately billed, regardless of age or concession card status.

Missed Appointment & Cancellation Policy

Cancellations must be made at least 1 hour prior to your scheduled appointment time. Non-attendance or failure to cancel with adequate notice may incur a $40.00 missed appointment fee.
